New Patient Information
Getting Ready for Your First Visit
- Please arrive 15 minutes early for your first visit to complete intake paperwork, or complete your forms in advance if online intake is available.
- Bring a valid photo ID, your insurance card, and a list of any current medications, supplements, or vitamins.
- If you have records, imaging, or test results from a previous provider, bringing them (or having them sent in advance) helps Dr. Rajah get the full picture from day one.
- If you're coming in for a specific concern, jot down when your symptoms started and how they've changed over time. That timeline is often the most useful thing you bring to an evaluation.
Telemedicine
Telemedicine Visits
Virtual appointments will be available for established and new patients located throughout California. Telemedicine is a good fit for:
- Follow-up visits and medication management
- Chronic neurological condition check-ins
- Reviewing test results and diagnostic findings
- Wellness and lifestyle consultations
To join your telemedicine visit: you’ll receive a secure link via email or text prior to your appointment. Click the link at the time of your visit from your phone, tablet, or computer – no special software required.

Insurance & Billing
We accept a variety of insurance plans, and also offer cash pay and hybrid/concierge payment options. For questions about your coverage or a bill, please contact our office directly.
Referrals & Care
When additional imaging, hospital-based care, or specialist consultation is needed, our team coordinates the referral and stays in communication with your care team.

Prescription Refills
For routine prescription refills, please contact our office at least 72 hours before your medication runs out. Certain medications may require an office or telemedicine visit before a refill can be authorized.
After-Hours Care
For medical emergencies - including sudden stroke symptoms, seizures, or loss of consciousness - call 911 or go to your nearest emergency room immediately.
